Evaluation of Uterosacral Ligament Involvement in Deep Endometriosis by Transvaginal Ultrasonography

This surgical-histopathology study evaluated transvaginal ultrasonography (TVS) for diagnosing uterosacral ligament (USL) involvement in 118 women with deep infiltrating endometriosis, with all scans performed within 2 weeks of surgery by a single trained, blinded examiner. TVS identified 84 cases of USL endometriosis, and 81 of these were confirmed at the surgical/histopathology gold standard, yielding sensitivity 95.3%, specificity 90.9%, PPV 96.4%, NPV 88.2%, and accuracy 94.1%. The authors also described four USL lesion ultrasound morphologic types (thickened/stiff, local nodules, irregular striped, and mixed), based on features seen in confirmed cases, and reported false negatives related to missed bilateral disease or multiple adherent retrocervical lesions and false positives due to USL tumor or inflammatory lesions.
