Abstract

虫垂子宮内膜症による虫垂重積症は比較的稀な疾患であり，虫垂腫瘍との鑑別が困難である．今回われわれは術前に虫垂腫瘍と診断し，単孔式腹腔鏡下回盲部切除術を施行した虫垂子宮内膜症による虫垂重積症の1例を経験したため報告する．症例は40歳代女性．便潜血反応陽性の精査目的で施行した下部消化管内視鏡検査で虫垂開口部の腫大と管腔側への突出を認めた．CTでは盲腸背側に明らかな充実部を伴わない径25mm大の嚢胞性病変を認め，虫垂腫瘍の可能性が否定できなかったため，単孔式腹腔鏡補助下回盲部切除術・D3郭清を施行した．病理組織学的診断は虫垂子宮内膜症による虫垂重積症であった．術前に症状や画像検査から虫垂腫瘍と虫垂子宮内膜症による虫垂重積症を鑑別するのは困難であるが，閉経前女性の虫垂病変においては子宮内膜症の可能性も考慮する必要がある．
