Concomitant autoimmunity may be a predictor of more severe stages of endometriosis

This matched case–control study evaluated 384 women with surgically diagnosed endometriosis (stages I–IV by r-ASRM) to test whether concomitant autoimmune disease was associated with more advanced disease. Women with endometriosis plus autoimmune disease (n=96) had higher mean ASRM scores than those without autoimmunity (51 vs 39) and were more likely to have stage IV endometriosis while being less likely to have stage III, with age, timing of laparoscopy, and hormonal therapy use comparable between groups; univariate and multivariate logistic regression found concomitant autoimmunity to be a significant predictor of stage IV (multivariate OR 2.54, 95% CI 1.57–4.10). The authors explicitly note that causality cannot be inferred and that the study cannot determine whether autoimmunity drives endometriosis severity or reflects shared mechanisms. This paper is centrally about endometriosis — it tests whether concomitant autoimmune disease predicts more severe (stage IV) endometriosis.
