Abstract

症例は29歳，女性．右下腹部痛を主訴に前医を受診した．CTで腹腔内膿瘍を伴う急性虫垂炎と診断され，interval appendectomyの方針で抗菌薬加療が行われた．症状は一旦改善したが，その後，短期間で2回の再燃を繰り返したため当院に紹介となり，腹腔鏡下虫垂切除術を施行した．病理組織学的に，腫大した虫垂の筋層から漿膜下層にかけて子宮内膜腺の増生とCD10陽性の間質を認め，虫垂子宮内膜症の診断となった．虫垂子宮内膜症は非常に稀な疾患で，無症候性のこともある．有症例であっても月経周期との関連が乏しかったり，さらに特異的な画像所見もないため，術前診断を得ることは難しい．自験例のように生殖年齢の女性で虫垂炎を繰り返す場合は，虫垂子宮内膜症の可能性も念頭に置き治療に臨む必要がある．
