Abstract
症例は, 49歳の女性。下腹部痛を主訴として来院。来院時, WBC15200, CRP25-2。腹部は膨満し, 右下腹部に強い圧痛とBlumberg兆候を認め, 急性虫垂炎の診断で緊急手術を施行した。術中所見では, 虫垂は著明に発赤腫大し, 虫垂先端にゼリー状の付着物を認めた。手術時, 回盲部への炎症が強く波及していたため, 回盲部切除および腹腔ドレナージを施行した。虫垂先端部の病理所見では, 筋層内から漿膜下組織にかけて円柱上皮からなる腺管と紡錘細胞からなる間質が散在してみられ, 子宮内膜症と診断した。
