Synchronous endometrial and ovarian carcinomas: predictors of risk and associations with survival and tumor expression profiles

This population-based study examined predictors of synchronous endometrial and ovarian cancers (SEOs) and compared overall survival between 52 patients with SEO tumors and 293 patients with endometrioid or clear cell ovarian carcinomas, using medical record abstraction, tumor tissue microarrays, unconditional logistic regression, and Cox models. Endometriosis at the ovary was the strongest multivariable predictor of decreased SEO risk (OR 0.45), while premenopausal status and lower pre-treatment CA125 showed weaker, non-significant associations. Patients with SEOs did not have significantly different survival versus those without SEOs, but SEO tumors more frequently showed deficiency of MLH1, PMS2, and PTEN compared with endometrioid ovarian carcinoma. This paper relates to endometriosis and adenomyosis because it finds an ovarian endometriosis association with SEO risk and evaluates molecular differences in synchronous endometrial–ovarian cancers.
