Abstract

症例は64歳の女性で，腹痛と嘔吐を主訴に当院を受診した．CTおよび下部消化管内視鏡で上部直腸の腫瘍性狭窄を認めた．生検病理では腸管原発よりも婦人科臓器原発を考える腺癌と診断された．本症例は，39歳時に子宮内膜症のため子宮全摘と右卵巣切除を施行されていた．腹腔鏡下直腸低位前方切除術，D3郭清，左卵巣合併切除を施行した．手術病理は直腸子宮内膜症関連腺癌と診断された．術中に発見，切除した肝表面の結節は腺癌の転移と診断され，リンパ節も多数が転移陽性であった．術後は，パクリタキセル・カルボプラチン・ベバシズマブによる化学療法を6コース施行した．術後1年3カ月でリンパ節転移，肝転移が出現したため，化学療法を再開している．腸管子宮内膜症からの癌化は稀であるので，若干の文献的考察を加えて報告する．
