A Case of Endometrioid Adenocarcinoma Arising from Endometriosis of the Sigmoid Colon Concurrently with Ovarian Endometrioid Adenocarcinoma

This case report describes a 46-year-old woman admitted for abdominal pain with a ruptured right ovarian tumor and imaging findings suggestive of malignancy (right ovarian cystic tumor ~8 cm) alongside a sigmoid colon type 2 lesion seen on colonoscopy. Surgery was performed with an intraoperative impression of sigmoid colon cancer and ovarian cancer; histopathology identified endometrioid adenocarcinoma in both sites, and additionally showed transitional areas from endometrial-type tissue to carcinoma in each lesion, leading the authors to diagnose synchronous cancers arising from ectopic endometriosis of the sigmoid colon concurrent with ovarian endometrioid adenocarcinoma. Periintestinal lymph node metastases were present, but the patient achieved 5-year survival following postoperative adjuvant chemotherapy, and the authors note that malignant transformation of intestinal endometriosis is rare with no prior reports of simultaneous cases with ovarian cancer.
