This review studied how endometriosis affects female fertility and pregnancy, synthesizing literature from PubMed and the 2022 ESHRE guidelines, with emphasis on newer meta-analyses. It reports multifactorial mechanisms—systemic inflammation with embryotoxic cytokines, impaired sperm survival/motility, altered endometrial receptivity/progesterone resistance, and possible tubal/ovarian dysfunction—along with evidence for reduced oocyte/embryo quality and, after endometrioma surgery, lower ovarian reserve markers (AMH/AFC). Major caveats are the inhomogeneity and variability of outcomes across included studies (including pregnancy course data), limited/uncertain estimates for tubal disease incidence, and that direct therapeutic implications for some associations (e.g., miscarriage risk) are not clearly derived. The paper is centrally about endometriosis — it reviews mechanisms of reduced fertility, effects during early pregnancy, and pregnancy outcomes, including links to adenomyosis where co-occurrence is discussed.
