This paper studied whether peritoneal fluid concentrations of leptin and interleukin-15 (IL-15) are associated with the severity of pelvic endometriosis. Peritoneal fluid was collected during laparoscopy from 40 women with endometriosis and 15 control women without endometriosis, and PF leptin and IL-15 were quantified using enzyme immunoassay. The authors found that both PF leptin and IL-15 were significantly higher in the endometriosis group than controls and differed across endometriosis stages, with higher leptin in stages I–II than III–IV and higher IL-15 in stage I than III/IV; they also reported significant negative correlations between each cytokine and disease stage, and a positive correlation between leptin and IL-15 within the endometriosis group. This paper is centrally about endometriosis — it links elevated peritoneal fluid leptin and IL-15 with pelvic endometriosis severity and early-stage disease.

Abstract

Objective: This study investigated the relation between peritoneal \nfluid (PF) leptin and interleukin-15 (IL-15) concentrations and the \nseverity of pelvic endometriosis. Design: Peritoneal fluid samples were \ncollected from 40 women with endometriosis and 15 women without \nendometriosis as controls. Setting: The Department of Obstetrics and \nGynecology, Benha Faculty of Medicine, ZagazigUniversity. Intervention: \nPeritoneal fluid samples were collected during laparoscopy. \nQuantitative measurement of PF leptin and IL-15 was done by enzyme \nimmunoassay. Results: The mean ranks of PF leptin and IL-15 were \nsignificantly higher in women with endometriosis compared to controls \n(p = 0.001 and 0.004, respectively). PF leptin and IL-15 levels were \nsignificantly different in the various stages of endometriosis. The PF \nleptin in stages I and II was significantly higher than stages III and \nIV. In addition, significantly higher concentrations of IL-15 were \nfound in stage I than stage III and IV. A significant negative \ncorrelation was found between PF leptin levels and the stage of \nendometriosis (r = -0.578, p < 0.001). Moreover, a significant \nnegative correlation was found between IL-15 and the stage of \nendometriosis (r = -0.425, p < 0.001). Furthermore, a significant \npositive correlation was found between PF leptin and IL-15 levels in \nendometriosis group (r = 0.381, p < 0.05). Conclusion: This work \ndemonstrated that PF leptin and IL-15 concentrations were elevated in \nwomen with endometriosis particularly in the early stages of the \ndisease. This could suggest a role for leptin and IL-15 in the \npathogenesis of peritoneal endometriosis and development of endometrial \nperitoneal implants.
