Abstract

比較的稀な鼠径部子宮内膜症の1例を経験した.症例は40歳女性で,月経に一致した右鼠径部の疼痛を伴う腫瘤を認めた.病歴と画像診断より本症を疑い手術を行った.腫瘤は子宮内索より発生しており,広範囲にわたる腫瘤摘出術を施行した.病理組織学的所見で子宮内膜症と確定診断した. 1年3カ月後の現在に至るまで再発を認めていない.本症の本邦報告例は自験例を含め41例で,その好発年齢は平均で39.0歳,右側に多い傾向がある.術前に本症と診断したのは32%で,月経に一致した疼痛を伴う腫瘤という病歴がその診断に有効である.治療はほぽ全例に腫瘤摘出術が行われているが, 1例に再発を認めている.手術に際しては取り残しのない広範囲の腫瘤摘出術が重要であると考えられた.本症例の概略と41例の集計に検討を加え報告する.
