Abstract

26歳の女性で，4年前より子宮内膜症既往がある．1年前から左鼡径部に月経周期に伴い症状が変化する有痛性腫瘤を自覚していた．CT，MRIでは左鼡径部に辺縁に造影効果のある多房性嚢胞を認めた．左鼡径部の外性子宮内膜症を疑った．全身麻酔下に右卵巣内膜症性嚢胞に対して腹腔鏡下子宮内膜症病巣除去術と鼡径部腫瘤摘出手術を行った．腹腔内から左内鼡径輪開存を観察し，前方アプローチにて鼡径管を開放すると暗赤色の腫瘤をヘルニア嚢内に確認した．ヘルニア嚢を高位結紮し，Marcy法にてヘルニア門を閉鎖した．病理組織学的検査では，ヘルニア嚢内に子宮内膜症の組織像を認めた．術後12か月経過後も左鼡径部腫瘤の再発は認めていない．本邦での鼡径部外性子宮内膜症160例を集計したところ，ヘルニア嚢内に発生したのは34例であった．女性の鼡径部腫瘤の診断の際には，鼡径ヘルニアの鑑別診断の一つとして本症も考慮すべきであると思われた．
