Interstitial cystitis and endometriosis in patients with chronic pelvic pain: The "Evil Twins" syndrome.

This prospective study evaluated 178 women aged 18–60 years with chronic pelvic pain and both uterine and bladder/anterior vaginal wall tenderness, using concurrent laparoscopy with histologic confirmation of endometriosis and cystoscopy with bladder hydrodistention to diagnose interstitial cystitis (IC) based on NIH-NIDDK criteria. Endometriosis was found in 134 (75%) patients and IC in 159 (89%), with both diagnoses present in 115 (65%), while 19 (11%) had endometriosis without IC and 44 (25%) had IC without endometriosis; the authors report a major limitation that participants were selected specifically for both uterine and bladder tenderness, likely inflating overlap versus the general CPP population. The potassium sensitivity test (PST) was positive in 146 (82%) and correctly predicted cystoscopic IC in 96% of PST-positive patients, although a minority of cystoscopic IC cases occurred with a negative PST. This paper is centrally about endometriosis — it prospectively quantifies overlap between endometriosis and interstitial cystitis in women with chronic pelvic pain using laparoscopy and cystoscopy.
