Comparison of Clinico-Pathological Features Between Epithelial Ovarian Cancer Patients With and Without Endometriosis: A Cross-Sectional Study
Epithelial ovarian cancer patients with endometriosis were younger and had fewer pregnancies and births, with a higher proportion having clear cell or endometrioid histopathology and unilateral tumors compared to those without endometriosis.

This cross-sectional study compared demographic and obstetric characteristics and key clinico-pathological features between epithelial ovarian cancer patients with endometriosis-associated ovarian cancer (EAOC) and those without endometriosis at Al-Zahra hospital (2011–2017), analyzing 20 EAOC cases versus 140 non-EAOC patients undergoing surgery. EAOC patients were significantly younger and had fewer pregnancies, lower parity, and fewer term pregnancies, and their tumors more often showed clear cell and endometrioid histopathology with a higher proportion being unilateral. The study’s main limitation is the small EAOC sample size (n=20) relative to the non-EAOC group, alongside its cross-sectional design. This paper is centrally about endometriosis — it directly compares clinicopathological differences between ovarian epithelial cancer patients with and without endometriosis (EAOC).
