Abstract

症例は34歳，女性．開腹歴はなく，30歳時に子宮内膜症の診断を受けたことがあった．腹痛を主訴に前医を受診し，腸閉塞の診断で入院となり，イレウス管による保存的治療が施行された．小腸造影検査で回腸末端に狭窄像を認め，骨盤MRI検査では腸管子宮内膜症による小腸狭窄が強く疑われた．腸閉塞症状が軽快した後，精査加療目的で当院に紹介となった．回腸子宮内膜症に対してホルモン療法後に，腹腔鏡下手術を施行した．腹腔内を観察すると後膣円蓋と直腸前壁に子宮内膜症による軽度の癒着を認めた．また，回腸末端部より約20cmの回腸に漿膜の引きつれと硬化を認め，小腸部分切除術を施行した．病理組織検査では，狭窄を認めた回腸の粘膜下層から漿膜下層に子宮内膜腺および間質細胞が分布しており，回腸子宮内膜症と診断した．術後経過は良好で第９病日に退院となった．
