Abstract

症例は47歳女性．2018年10月に腹痛で当院救急外来を受診した．造影CTで回腸終末部の壁肥厚を閉塞起点とした機械性腸閉塞を指摘され，保存的加療で改善した．2018年11月，同様の症状で再度当院を受診し造影CTで先の入院時と同じ所見を認め，再入院となった．下部消化管内視鏡検査で回腸壁の肥厚部の生検を行ったが診断に至る所見は認めなかった．腸閉塞症状を繰り返すことより手術適応と判断，2019年２月に手術を施行した．腹腔鏡にて腹腔内を観察すると回腸漿膜面の赤色結節と強く屈曲した腸管を認めたため，腹腔鏡下回盲部切除術を施行した．術後経過は良好で合併症なく退院した．標本の病理検査で異所性子宮内膜症の所見を認めたため回腸子宮内膜症に伴う腸閉塞と考えられた．子宮内膜症はしばしば腸管に生じ腸閉塞などの症状をきたしうるが，回腸に発生した腸管子宮内膜症は比較的稀であり，若干の文献的考察を加え報告する．
