鼡径部に発生した稀な子宮内膜症の症例を経験したので報告する. 症例は未婚・未経産婦の33歳の女性.左鼡径部の腫瘤と疼痛を主訴に外来受診.左鼡径部に直径10 mm の圧痛を伴う腫瘤を認め, 月経時に症状の増悪を認めた.子宮内膜症を合併した鼡径部腫瘤の診断で手術を行った.手術所見では外鼡径輪の近傍に直径10 mm 大の腫瘤を認めた.病理組織学的検討では, 組織内に子宮内膜組織が混在しており外性子宮内膜症と診断した.

右横隔膜ヘルニアに対し小開胸を併用した胸腔鏡下手術でヘルニア門を縫合閉鎖した.この症例は2年前月経随伴性気胸で胸腔鏡下の手術を施行していた.初回手術では, 横隔膜腱様部の2個の病変部を電気凝固し, さらに右肺中葉の1個の小ブラも切除した.気胸の再発はなかったが, 初回手術後3ヵ月に凝固した場所に一致して横隔膜ヘルニアを発生し次第に増大したため手術を施行した.脱出臓器は肝臓で, ヘルニア門は30mm径であった.初回手術時に病変部の切除と縫合閉鎖をすべきであったと考えられた.
