This ecological, descriptive quantitative study analyzed secondary hospitalization data from SIH/DATASUS to characterize endometriosis-related morbidity, mortality, sociodemographic distribution, and SUS financial impact in Southeastern Brazil from 2015 to 2024. Across 53,105 hospitalizations, cases were concentrated in São Paulo and Minas Gerais, and the largest age group was 30 to 49 years, consistent with delayed diagnosis into late reproductive stages; overall mortality was low (0.15) but Black women showed higher proportional lethality (0.11%) than Pardo and White women, alongside total costs exceeding R$ 32.5 million. The study’s major caveat is that it relies on hospitalizations recorded in administrative datasets, which may reflect access to hospital care and coding rather than true disease prevalence.
