Pharmacological correction of immunometabolic disorders in patients with endometrioma along with chronic inflammatory diseases of the genital tract

The study evaluated inflammatory and immunometabolic biomarkers in 90 patients with ovarian endometriomas in the setting of a chronic inflammatory process of the small pelvic organs, compared with 30 gynecologically healthy women, using clinical-laboratory testing and immunoassays for cytokines (TNF-α, IL-1β, IL-4, IL-10, IFN-γ), immunoglobulins, C-reactive protein, neopterin, and vitamin D3. Patients receiving a basic regimen (cefotaxime, metronidazole, B vitamins, and combined oral contraceptives containing dienogest) were compared with an “main” regimen that added anti-inflammatory/antibiotic therapy plus agents aimed at improving microcirculation and having systemic/local anti-inflammatory and thrombolytic properties, together with dienogest and vitamin D3. Endometrioma patients showed increased pro-inflammatory cytokines and IFN-γ, markedly elevated C-reactive protein and increased neopterin versus controls, and the proposed program normalized IgG and reduced pro-inflammatory cytokine levels within one month; the paper’s limitation is that outcomes were assessed over a short one-month period.
