Abstract

症例は33歳，女性．月経周期に一致する下腹部痛を繰り返していた．不妊治療中の前医で腹部CTを施行されS状結腸に腫瘤像を認めたため精査加療目的に当院を紹介受診となった．超音波内視鏡下穿刺吸引生検（endoscopic ultrasound guided fine needle aspiration；EUS-FNA）で腸管子宮内膜症と診断し腹腔鏡下手術を行った．手術は，骨盤内腹壁に散在するblueberry spotに対し焼灼術を行うとともに，右卵巣囊腫摘出術を施行した．S状結腸は腫瘤状の内膜症病変による引きつれと屈曲による狭窄を認め，S状結腸切除術を施行した．病理組織学的検査ではS状結腸の粘膜下層から漿膜下層に子宮内膜腺および間質細胞を認めた．
