This retrospective study analyzed 217 infertile patients with ovarian endometrioma who underwent laparoscopic cystectomy between January 2014 and October 2019 to assess whether preoperative neutrophil-to-lymphocyte ratio (NLR) predicts natural pregnancy within 1 year. Using Cox regression and multivariate models, the authors found that patients with natural pregnancy had significantly higher preoperative NLR than those with pregnancy failure, and that age, least function (LF) score, and NLR were independent predictors; a nomogram-based NLR/LF/age model showed moderate discrimination (AUC = 0.725). Patients were stratified into low-, intermediate-, and high-risk groups with 1-year natural pregnancy rates of 43.5%, 34.4%, and 21.0%, respectively. The authors’ key caveat is that the analysis is retrospective in design. This paper is centrally about endometriosis — it specifically examines whether preoperative NLR predicts postoperative natural fertility outcomes in infertile patients with ovarian endometrioma.
