This paper reports a 43-year-old woman with clear cell adenocarcinoma arising from malignant transformation of abdominal wall endometriosis in a surgical scar 19 years after cesarean section, with preoperative diagnosis via needle biopsy and staging using whole-body simultaneous 18F-FDG PET/MR on a 3.0T PET/MR scanner. PET/MR identified a large solid heterogeneous abdominal wall mass with high FDG uptake (SUVmax 9.61) and additional FDG-avid retroperitoneal and inguinal lymph nodes, while showing no abnormal uterine or ovarian FDG uptake; histopathology confirmed stage IIIC clear cell adenocarcinoma originating in ectopic endometrial tissue with 51 of 69 lymph nodes positive. The main limitation is that the work is a single-case “Image of the Month” report, so findings cannot establish diagnostic accuracy or generalizability.
