Predictive value of preoperative MRI using the #ENZIAN classification score in patients with deep infiltrating endometriosis

This retrospective study evaluated how well preoperative abdominopelvic MRI findings, scored using the #ENZIAN classification, predicted intraoperative findings in 64 patients undergoing surgery for deep infiltrating endometriosis. Operative scores served as the gold standard, and sensitivity, specificity, and positive and negative predictive values were calculated for each #ENZIAN compartment. MRI showed the highest performance for compartments O (ovarian lesions), A (rectovaginal septum and posterior vaginal fornix), and B (uterosacral ligaments and parametrium), with near-perfect sensitivity and specificity in O and A, while compartment P (peritoneal lesions) had low sensitivity (14%) and low PPV (7%). Limitations include its retrospective design and the single-center sample without disclosed funding or detailed external validation.
