Effect of Pelvic Endometriosis, Endometriomas and Recurrent Endometriomas on IVF-ET/ICSI Outcomes
Pelvic endometriosis negatively impacted ovarian reserve less than endometriomas, and transvaginal aspiration did not improve IVF outcomes for recurrent endometriomas.

This retrospective study evaluated ovarian response to GnRH-a–based COH and IVF-ET/ICSI outcomes in 380 women with endometriosis after prior laparoscopy, comparing four groups defined by presence of endometriomas and whether recurrent endometriomas were aspirated transvaginally before COH. It found that baseline FSH and stimulation characteristics differed, with recurrent endometrioma cases after cystectomy (particularly the aspiration group) showing higher baseline FSH (lower ovarian reserve indicators), while numbers of high-quality embryos and transfer embryos were similar across groups and implantation and pregnancy rates did not differ significantly. The paper’s key caveat is its retrospective design and group differences in baseline stimulation parameters (e.g., FSH, total gonadotropin dose, induction duration, estradiol on hCG day), despite similar demographic variables. Relevance to endometriosis: this study directly compares how pelvic endometriosis versus (recurrent) ovarian endometriomas—and transvaginal aspiration for recurrent endometriomas—relate to IVF/ICSI ovarian response and pregnancy/implantation outcomes.
