Association between ovarian endometriosis and malignancy in the peri-menopausal period: report of two cases and review of the literature

This paper reports two peri-menopausal case reports in which ovarian endometriosis was associated with ovarian malignancy, identified in a single institution between 2001 and 2007. The first patient (49 years) had an endometrioid cyst previously managed laparoscopically and later underwent restaging after definitive histology revealed endometrioid adenocarcinoma with foci of clear cell carcinoma; the second patient (49 years) underwent surgery after imaging suggested endometriosis and subsequently had frozen section showing adenocarcinoma, with definitive histology of ovarian carcinosarcoma containing areas of endometrioid adenocarcinoma and nearby endometriosis. Key findings were that such associations occurred in about 1.29% of laparoscopies for ovarian endometriosis in that age group, and both patients were disease-free during follow-up after surgery and chemotherapy. A major limitation is that the work is based on only two cases with follow-up and concludes that more data are needed to determine whether aggressive versus conservative management is warranted in peri-menopausal endometriosis. This paper is centrally about endometriosis — specifically, it documents peri-menopausal ovarian endometriosis cases complicated by ovarian cancer and reviews the literature on the association.
