Clinical Presentation and Management of Endometriosis-Related Hemorrhagic Ascites: A Case Report and Systematic Review of the Literature

This paper reports a single new case of a 34-year-old woman with irregular dysmenorrhea, massive hemorrhagic ascites, markedly elevated CA-125, and imaging findings suspicious for malignant ovarian tumor; laparotomy drained 2 liters of dark-red ascites and histology of implants confirmed stage IV endometriosis. The authors then systematically reviewed all human case reports/series (biopsy-proven endometriosis) of hemorrhagic/“brown/chocolate” ascites through December 2018, yielding 73 reports including 84 women, and extracted demographics, symptoms, operative findings, management, and outcomes. Across the literature, abdominal distension was most common, ascitic volumes were often massive (mean ~4.2 L), most patients had moderate-to-severe endometriosis (ASRM III–IV), and CA-125 was frequently elevated; recurrence was reported in 36.9%, but was lower (8.3%) after definitive surgery with ovarian function suppression, with recurrence sometimes occurring after stopping GnRH. Limitations explicitly include uncertainty about pathogenesis (given that ovarian cyst rupture was reported in only a minority) and short median follow-up (8 months).
