The paper investigated whether dienogest’s antifolliculogenic effects involve suppression of gonadotropins or a direct ovarian action, using female cynomolgus monkeys given a single oral 0.1 mg/kg dose on day 7 of the menstrual cycle. Plasma FSH, estradiol (E2), and progesterone were followed for up to 15 days, and ovaries were excised 24 h after dosing for histology. Dienogest caused a rapid decline in plasma E2 within 24 h while FSH levels did not decrease prior to the E2 drop, with sustained low E2 for more than 11 days; histology showed apoptotic changes in granulosa cells of the dominant follicle in 2 of 3 animals. The authors’ limitation is the small histological sample size (two responders out of three), leaving mechanistic conclusions constrained.
