Abstract

重積を伴う虫垂子宮内膜症は稀な疾患で, 医学中央雑誌で検索しえた限り6例目である。症例は月経周期と関連のない右下腹部痛を主訴とする43歳女性で, 大腸内視鏡検査では虫垂開口部に一致して発赤を伴う盲腸側に突出した粘膜下腫瘍様の隆起を認め, 注腸検査では虫垂は描出されず, 盲腸に4cm大1/3周性の陰影欠損を認めた。腹部CT検査では盲腸から虫垂に一致して4cm大の腫瘤影を認めた。虫垂腫瘍が疑われ, 腹腔鏡補助下に回盲部切除術を施行した。虫垂は盲腸, 回腸末端と癒着して一塊となり後腹膜に癒着固定されていた。切除標本では腫大した虫垂が盲腸内に重積し, 直径25mm大の腫瘤を形成していた。病理組織学的には盲腸内に重積した虫垂の漿膜筋層に子宮内膜症様組織を虫垂子宮内膜症と診断された。本症は良性疾患で薬物療法もあるが, 正診率は極めて低く有症状例では最初から手術療法が選択されてもよいと考えられた。
