Abstract

子宮内膜症は全身の臓器に起こりうるが後腹膜腔は頻度が少なく，癌化はさらに稀である．症例は42歳の1経妊1経産婦で，月経時の右上腹部痛を主訴に受診し，腹部CTで後腹膜腫瘍を指摘された．MRIで上行結腸の外背側に，内部に液体成分と充実性成分を伴う10×9cm大の囊胞性腫瘤を認めた．CA19-9は2,120U/mL，CA125は116U/mLと上昇を認めた．後腹膜腫瘍摘出術を施行，腫瘍は他臓器への浸潤はなく摘出できた．病理所見では茶褐色液体を含む囊胞病変で，組織学的に婦人科癌に類似したadenocarcinomaの所見であり，総合的に子宮内膜症に関連した後腹膜悪性腫瘍と判断した．PET-CTで明らかな集積は認めず，診断も兼ねて子宮全摘+両側付属器切除+大網部分切除術を追加で施行したが，病理学的に悪性所見は認めなかった．術後，子宮内膜癌の術後化学療法に準じTC療法6コースを施行し，現在無再発経過中である．今回われわれは，子宮内膜症の悪性転化も考えられる組織像を呈した後腹膜腫瘍の1例を経験したので報告する．
