Gastric endometriosis associated with transverse colon endometriosis: a case report of a very rare event

This paper describes a 28-year-old woman with a 4-year history of abdominal pain with bowel symptoms and dyspepsia/epigastric postprandial pain that worsened during menses, in whom imaging and laparoscopy identified deep infiltrating endometriosis (DIE) involving a severely stenotic rectosigmoid, a 6-cm transverse colon stenosis, infiltration of the gastrocolic ligament, and a ~6-cm induration of the stomach antrum. Double-contrast barium enema and pelvic MRI guided the workup, while colonoscopy was not possible due to the rectosigmoid narrowing; definitive diagnosis came from surgical histology and CD-10 immunohistochemistry showing endometriotic stromal cells, with gastric involvement of all layers except the mucosa. A major caveat is that the case report cannot determine which lesion invaded first, and diagnosis relied on resected tissue and differential workup of submucosal/intramural gastric tumors.
