Abstract

Twenty-six women, most of them with advanced endometriosis (Stage III according to Acosta), were treated for six months with the LH-RH agonist Buserelin. After initially increased liberation both of the gonadotropins as well as of the sexual steroids there was consecutive inhibition of gonadal function, especially after inhibition of FSH secretion. Within two to four weeks estradiol production was completely and lastingly suppressed in 21 of the 26 women. In three women it lasted for three months and in two women estradiol again rose up to three times during the period of observation. Correspondingly, 22 of the women suffered from hot flushes as early as within the first two months. Twenty of the 26 women became completely free of symptoms within four to six weeks, and a clear regression of the pain was recorded in another four. In two women the complaints persisted without change. Withdrawal bleeding of short duration occurred simultaneously with the intermittent increases in estradiol levels; however, this bleeding usually caused little or no pain. Eleven of the 19 endometrium biopsies performed during therapy revealed a dormant endometrium, and only in one case was proliferated endomedium found, in the third month of treatment, in a stage of transition to hyperplasia. The remaining seven biopsies were insufficient for assessment. In seven women with advanced endometriosis, laparotomies were performed after drug treatment. Histologic study of the endometriosis tissue thus obtained revealed, except in the two nonresponders, cicatrized tissue with atrophic glands and severely reduced stroma.(ABSTRACT TRUNCATED AT 250 WORDS)

Nach anfänglicher verstärkter Freisetzung sowohl der Gonadotropine als auch der Sexualsteroide kam es, insbesondere nach Inhibition der FSH-Sekretion, zur konsekutiven Hemmung der Gonadenfunktion. Innerhalb von 2-4 Wochen war bei 21 der 26 Frauen die Östradiolproduktion vollständig und anhaltend unterdrückt. Bei 3 Frauen dauerte es 3 Monate, und bei 2 Frauen stieg innerhalb des Beobachtungszeitraumes die Östrogenproduktion noch bis zu dreimal an. Dementsprechend kam es bei 22 Frauen schon innerhalb der ersten beiden Monate zum Auftreten von Hitzewallungen. 20 der 26 Frauen wurden innerhalb von 4-6 Wochen völlig beschwerdefrei, bei weiteren 4 konnte ein deutlicher Rückgang der Schmerzen registriert werden. Bei 2 Frauen blieben die Beschwerden unverändert bestehen. Parallel zu den intermittierenden Östradiolanstiegen traten kurzzeitige Entzugsblutungen auf, die jedoch meist nicht oder nur gering schmerzhaft waren. 11 der 19 Endometriumbiopsien, die unter der Therapie durchgeführt worden waren, zeigten ruhendes Endometrium, und nur in 1 Fall fand sich im 3. Behandlungsmonat proliferiertes Endometrium im Übergang zur Hyperplasie. Die restlichen 7 Biopsien waren für eine Beurteilung unzureichend. Bei 7 Frauen mit einer fortgeschrittenen Endometriose wurde nach der medikamentösen Behandlung eine Laparotomie durchgeführt. Das dabei gewonnene Endometriosegewebe ergab histologisch, bis auf die beiden Therapieversager, ein Narbengewebe mit atrophischen Drüsen und stark vermindertem Stroma. Bei den beiden Frauen, die unter der Behandlung weiterhin Beschwerden angaben, fanden sich sowohl proliferierte als auch atrophische Gewebeanteile in ein und demselben Endometrioseknoten. Ovarbiopsien (n = 3) bei Frauen mit supprimierter Gonadenfunktion zeigten neben zahlreichen Primordialfollikeln nur wenige Sekundär- und keine Tertiärfollikel als Zeichen der ausgeprägten Follikelreifungsstörung. Ernsthafte Nebenwirkungen unter der Buserelintherapie wurden nicht angegeben. Das Zyklusverhalten nach der Behandlung zeigte eine Tendenz zur verzögerten Wiederaufnahme der Ovarialfunktion, was auf die langdauernde Hemmung der hypophysären und gonadalen Funktion zurückzuführen ist. Die weitere Zyklusfunktion war jedoch ungestört. Aufgrund der guten Verträglichkeit war die Akzeptanz groß. Ein bis 3 Monate nach Einsetzen der Ovarialfunktion bzw. 2-6 Monate nach Therapieende kam es allerdings bei den rein medikamentös behandelten Frauen zum Wiederauftreten der Schmerzsymptomatik, woraus zu entnehmen ist, daß bei fortgeschrittenen Fällen mit Buserelin wie mit anderen hormonalen Therapeutika keine endgültige Heilung der Endometriose zu erwarten ist. In the two women who claimed that their complaints persisted under treatment, both proliferated and atrophic portions of tissue were found in one and the same endometriosis cyst! Ovary biopsies (n = 3) in women with suppressed gonadal function revealed, apart from numerous primordial follicles, only few secondary and no tertiary follicles as signs of a severe follicle maturation disorder. No serious side-effects of Buserelin therapy were complained of. The posttherapeutic cycle pattern showed a tendency toward delayed resumption of ovarian function; this is attributable to the long duration of inhibition of pituitary and gonadal function. However, there were no other disturbances of cycle function. Because of the good tolerance the degree of acceptance was high. However, among the women who had been treated with drugs only, there was a recurrence of the pain symptoms on to three months after onset of ovarian function or two to six months after discontinuation of therapy. It may be concluded from this that in advanced cases Buserelin cannot be expected to bring about definitive healing of the endometriosis, any more than other hormonal drugs can.
