Large, Non-Cavity Distorting Intramural Leiomyomas Decrease Leukemia Inhibitory Factor in the Secretory Phase Endometrium
Large, non-cavity distorting intramural leiomyomas are associated with decreased leukemia inhibitory factor in the secretory phase endometrium compared to controls.

This prospective cohort study examined whether large intramural leiomyomas (IM, ≥3 cm) affect endometrial implantation-factor expression during the window of implantation. The authors collected mid-secretory phase endometrial biopsies from sub-fertile patients with non–cavity-distorting IM (n=9) at planned myomectomy and compared them with controls from young normally menstruating women without leiomyomas (n=8), using quantitative immunohistochemistry to measure HOXA10, leukemia inhibitory factor (LIF), estrogen receptor-α (ER-α), and progesterone receptor (PR). Patients with large IM had significantly decreased LIF expression versus controls, while HOXA10 and PR did not differ and ER-α showed only a trend toward increase; the authors’ stated limitation includes the small sample size and the need for further work to determine whether LIF downregulation is corrected after resection. This paper directly relates to endometriosis and/or adenomyosis by measuring LIF in the implantation window and citing prior evidence that LIF is dysregulated in adenomyosis.
