This 1995 chapter reviews how multiple classification and staging schemes for endometriosis have evolved, noting that most systems were largely descriptive and based on the morphologic appearance of lesions, from which investigators attempted to infer pathogenesis and clinical consequences. It discusses the historical rationale for grouping patients into categories under the assumption that certain outcomes with and without treatment might be predicted, and it summarizes how subsequent work linked classification to clinical endpoints such as pain and infertility, including through revisions of widely used frameworks. A major limitation highlighted by the overall framing is that many systems originated from descriptive morphology and therefore depended on assumptions about how those appearances map to biology or prognosis. This paper is centrally about endometriosis — it focuses specifically on the history and principles of classification and staging of endometriosis.

Abstract

A multiplicity of classification schemes for endometriosis has evolved since clinical recognition of the disorder. Such systems, which have taken various forms, have been mostly descriptive based on the morphologic appearance of the disease. From these descriptions, investigators have made inferences regarding the pathogenesis and clinical consequences of this disorder. Attempts to group patients into categories were made with the presumption that certain outcomes with and without treatment could be predicted. Preview Unable to display preview. Download preview PDF.
