Endometriosis externa within the rectus abdominis muscle

This case report studied a rare presentation of endometriosis externa located within the rectus abdominis muscle in a 24-year-old woman with abdominal wall pain and a palpable mass, using ultrasonography, pelvic MRI, ultrasound-guided fine-needle aspiration biopsy, and excisional biopsy with histology and immunohistochemistry. Imaging identified a 23 mm heterogeneous hypoechoic suprapubic lesion with arterial blood supply, and cytology showed stromal and epithelial groups consistent with endometriosis externa, which was confirmed on excision by endometrial glands and stroma between skeletal and smooth muscle bundles. The authors note the overall diagnostic difficulty because imaging features are nonspecific and that definitive diagnosis relies on pathological examination.
