Susceptibility-weighted MRI of extra-ovarian endometriosis: preliminary results

This descriptive study evaluated susceptibility-weighted imaging (SWI) and fat-saturated T1-weighted imaging (fsT1WI) in eight pathologically proven cases of extra-ovarian endometriosis, involving bowel (n=3), bladder (n=2), and abdominal wall (n=3) lesions, acquired on a 1.5T MRI scanner. On fsT1WI, high-intensity foci consistent with subacute hemorrhage were seen in 4 of 8 lesions (50%), while SWI demonstrated signal voids consistent with acute to chronic hemorrhage in all 8 lesions (100%). The study’s main limitation is its very small, descriptive cohort with preliminary results, without larger validation or formal diagnostic accuracy testing. This paper is centrally about endometriosis—specifically extra-ovarian endometriosis and the use of susceptibility-weighted MRI to characterize hemorrhage within lesions.
