The Microbiome - Immune - Endometriosis Axis: Novel Insights into Pathogenesis and Therapeutic Perspectives

This narrative review synthesized and critically analyzed current literature on how the microbiome may contribute to endometriosis pathogenesis and to diagnostic and therapeutic possibilities, using PubMed, Scopus, Web of Science, and Google Scholar to cover original studies, systematic reviews, meta-analyses, and experimental work. The authors report that endometriosis is associated with microbiome composition changes across multiple niches (gut, vaginal, endometrial, and peritoneal), and propose mechanisms including altered estrogen metabolism via the estrobolome, dysregulated innate and adaptive immune responses, maintenance of chronic inflammation, activation of neuro-immune pain mechanisms, and epigenetic regulation of gene expression; they also describe effects on macrophages, natural killer cells, T lymphocytes, cytokine production, and signaling pathways linked to persistence and severity of endometriotic lesions. A major limitation acknowledged by the framing of the work is that it is a narrative synthesis rather than a single original experiment, so it does not itself resolve causal relationships or standardize heterogeneity across studies. This paper is centrally about endometriosis — it specifically reviews the microbiome–immune axis and summarizes potential microbiome-oriented diagnostic and therapeutic perspectives for endometriosis.
