Abstract

【症例】37歳女性．主訴は腹痛．３年前に虫垂子宮内膜症に起因する急性虫垂炎で腹腔鏡下盲腸部分切除術を施行された．その後黄体ホルモン療法を２年間行った後，不妊治療で凍結胚移植のため内服を終了したところ腸閉塞を発症，保存的加療で改善なく手術を施行した．回腸末端での癒着が強固であり，回盲部切除を施した．病理所見で回腸には複数か所で異所性子宮内膜が介在しており，その破綻に伴う癒着が腸閉塞の原因であった．【考察】腸管子宮内膜症は全子宮内膜症の12％に合併するとされる．その中でも直腸～S状結腸が72％と大部分を占め，小腸での発生は７％，虫垂では３％と報告されている．【結語】生殖年齢女性の腹痛の鑑別として腸管子宮内膜症が挙げられ，不妊治療による子宮内膜症治療中断がリスクとなる．腸管子宮内膜症の診断から治療，その後のフォローにいたるまで，消化器外科と産婦人科の連携が重要である．
