This case report describes a 39-year-old woman with a tender isolated abdominal wall tumor located in a Pfannenstiel incision, attributed to an endometrial tissue seeding deposit following a prior caesarean section, with no previously reported pelvic endometriosis. Clinically, the lesion mimicked an incarcerated incisional hernia, and the preoperative differential diagnosis included post-operative granuloma and the remote possibility of an incisional endometrioma (with no clear menstrual link), while local malignancy was not considered a serious possibility.
