Diagnostic accuracy of the IDEA protocol for non invasive diagnosis of rectosigmoid DE - a prospective cohort study

This prospective single-center cohort study evaluated the diagnostic accuracy of transvaginal ultrasound (TVS) using the IDEA protocol for clinically suspected rectosigmoid deep endometriosis, with surgical findings confirmed by laparoscopy and histology. Among 537 women analyzed, 279 (52%) had histologically verified rectosigmoid deep endometriosis, and TVS showed high performance (sensitivity 93.5%, specificity 94.6%, accuracy 94.04%, with LR+ 17.24 and LR− 0.07). The authors noted a limitation that additional pelvic pathologies were present in about 4–5% of women who met sonographic IDEA criteria but ultimately had other diagnoses, including Gartner duct cyst, anorectal abscess, rectal cancer, hydrosalpinx, metastatic endometrial cancer, and Crohn’s disease. This paper is centrally about endometriosis — it tests the IDEA protocol for non-invasive diagnosis of rectosigmoid deep endometriosis and reports differential diagnoses that can mimic ultrasound findings.
