Abstract

肛門部子宮内膜症は稀な肛門疾患で，分娩時に子宮内膜組織片が会陰切開創に移植することで発生する．今回，肛門部子宮内膜症の5例を経験した．初診時の年齢は30～43歳で，主訴は肛門の腫れと痛みであり，最終分娩から初診まで2～16年経過していた．全例に肛門前方の圧痛を伴う硬結を認めたが，皮膚の発赤や黒色変化を伴う皮膚隆起型が3例，皮膚表面には変化を認めない皮下硬結型は2例であった．肛門管超音波検査では低エコー腫瘤と描出され肛門管との連続は認めなかったが，皮下硬結型では肛門括約筋への浸潤を認めた．腫瘤切除手術は4例に行われたが，2例に再発を認め，ホルモン治療の追加が行われた．本症の診断に肛門管超音波検査は参考となりうる可能性がある．本症の皮膚隆起型と皮下硬結型では，理学的所見や肛門括約筋への浸潤度に違いを認めた．本症の治療は年齢，挙児希望，自覚症状，括約筋浸潤の程度などに基づいて選択するのが望ましい．
