Abstract

No evidence exists that medical treatment of endometriosis alleviates its associated subfertility. Medical treatment will render minimal and mild disease only temporarily invisible, allowing the lesions to re-emerge with time; surgical treatment can only remove visible lesions but will leave behind dozens, if not hundreds of invisible ones, which after removal of the visible lesions may develop into minimal (i.e. visible) endometriosis themselves and proceed from there. There is no rationale for treating occult endometriosis, and available treatments for minimal and mild endometriosis do not bring (theoretically and in practice) an enduring solution. In the past 20 years the role of medical treatment for relieving endometriosis associated subfertility has faded. In minimal and mild endometriosis medical treatment is no longer an option for patients suffering from subfertility. In patients in whom fertility is compromised by more complex endometriosis, it never has been a valid alternative to begin with.
