Abstract

症例は36歳女性．1年前に卵巣囊腫手術の際に子宮内膜症を指摘され，現在は不妊治療中だった．今回は月経時期に胸部違和感があり，前医で右気胸を指摘され当院へ紹介となった．気胸は初回で軽度だったが，本人の不妊治療の希望も考慮し診断も兼ねて月経期に胸腔鏡下手術の方針とした．胸腔内の所見は右胸腔全体に播種状の結節が多発していた．中葉の病変を部分切除し，横隔膜面とともに被覆し終了した．病理組織学的に結節は短紡錘形～類円形小型細胞を認め，各種免疫染色（CD10，ER，PgR）がそれぞれ陽性であり月経随伴性気胸と診断した．再発予防のため術後に癒着療法を施行し，現在術後2年半だが再発は認めていない．月経随伴性気胸を病理学的に証明できる例は少なく，今回は不妊治療中に発症した月経随伴性気胸において，鏡視下に播種状の子宮内膜組織が確認でき，それを病理学的にも診断し治療ができた1例を報告する．
