Two Young Women with Left-sided Pneumothorax Due to Thoracic Endometriosis

This case report describes two teenage/young women with left-sided pneumothorax occurring around menses, investigated with chest imaging and treated with emergency drainage followed by video-assisted thoracic surgery when air leakage persisted. In both patients, VATS revealed pleural and/or diaphragmatic nodules, and histopathology identified endometrial stroma with progesterone receptor expression (plus additional markers in the second case), leading to diagnoses of thoracic endometriosis; pelvic lesions were not seen on gynecologic evaluation. The paper notes that TE-associated pneumothorax can fall outside classic catamenial timing and may be difficult to diagnose without surgical exploration, and it also highlights uncertainty about TE mechanisms and prevalence, especially for rare left-sided juvenile presentations.
