Laparoscopic cystectomy of endometrioma: Good surgical technique does not adversely affect ovarian reserve

This prospective study enrolled 85 subfertile women undergoing their first laparoscopic endometriotic cystectomy using a stripping method, excluding those with prior pelvic surgery, and measured ovarian reserve before surgery and again on day 2 of the next menses. Ovarian reserve was assessed via real-time ultrasound (ovarian volume, antral follicle count, and stromal blood flow by Doppler) and serum day-2 FSH and LH, with ultrasound performed by a single observer and endometrioma severity staged clinically and by histology of the cyst wall. Among 73 with postoperative follow-up, there were no significant pre- vs postoperative differences in ovarian reserve parameters (ovarian volume, AFC, stromal peak systolic velocity, FSH, and LH), and histology-based follicle loss occurred in 27.2% of cyst walls, without a statistically significant association between endometriosis stage and follicle loss.
