Endometriosis presenting with recurrent massive hemorrhagic ascites and diagnosed by core needle biopsy
This case study describes a rare instance of recurrent massive hemorrhagic ascites caused by endometriosis, which was successfully diagnosed via core needle biopsy of the peritoneum.

This case report describes a 24-year-old nulliparous woman with recurrent massive hemorrhagic ascites and severe anemia who was diagnosed with endometriosis via core needle biopsy of the thickened peritoneum. The patient, who declined laparoscopy, experienced significant clinical improvement after receiving gonadotropin-releasing hormone analogs and oral contraceptives for eight months. The authors highlight that while endometriosis-related ascites is rare, core needle biopsy offers a safe and effective alternative to surgical diagnosis for identifying ectopic endometrial tissue in such presentations. This paper is centrally about endometriosis — specifically the diagnostic utility of core needle biopsy for cases presenting with massive hemorrhagic ascites.
