This review discusses diagnostic testing for endometriosis using serum and urine, focusing on the most promising single or combined biomarkers and comparing their reported sensitivity and specificity. It frames desired test characteristics as high sensitivity/specificity and positive/negative predictive value, along with reproducibility, low cost, and a preference for noninvasive and painless approaches, including potential use for monitoring response to medicinal therapy and for clinical study endpoints. A key limitation is that the work is a narrative overview of existing studies rather than a single standardized diagnostic evaluation. This paper is centrally about endometriosis — it reviews blood- and urine-marker tests for diagnosis and treatment monitoring in endometriosis.

Abstract A diagnostic test for endometriosis should exhibit high sensitivity and specificity as well as positive and negative predictive values. The test should be reproducible and have low costs. In addition patients would prefer a non-invasive and painless test. By means of this test it should be possible to achieve the first diagnosis of endometriosis as well as a measure of the success of a medicinal therapy of endometriosis. This would also be very important for the monitoring of clinical studies with new drugs. In this review article the most promising studies on single or combined tests on blood and urine samples are presented and the sensitivity and specificity are compared.
