Endometriosis of the appendix
This retrospective review of five appendiceal endometriosis cases found it presented variably, often undetected preoperatively, and should be considered in childbearing-aged women with acute abdominal pain.

This retrospective case series reviewed five patients diagnosed with appendiceal endometriosis at a single center among 2,173 appendectomies performed between 2007 and 2012, excluding patients treated for intra-abdominal endometriosis. Patients ranged from 26 to 60 years (mean 33.8), with presentations varying from asymptomatic incidental findings (two cases) to symptoms mimicking acute appendicitis, including right lower quadrant pain (two cases) and abdominal discomfort; imaging showed appendiceal abnormalities such as suspected mucocele or infected appendix, but no preoperative imaging suggested appendiceal endometriosis. None of the patients received adjuvant hormonal therapy, and diagnosis was based on final pathology with endometrial glands/stroma located in the appendiceal muscular/seromuscular or serosal layers. The paper is limited by the very small number of cases and its observational, single-institution retrospective design. This paper is centrally about endometriosis — specifically endometriosis of the appendix and its variable clinical/imaging presentation and pathology.
