This chapter reviews laparoscopic versus open laparotomy surgical approaches for treating endometriosis, contextualizing the uncertainty in prevalence, etiology, pathogenesis, and the controversial nature of therapy. It outlines that surgery can be conservative or radical, and performed either via laparotomy or operative laparoscopy, with medical and surgical options discussed more broadly. A major caveat explicitly emphasized is that symptoms and disease presentation are variable and that fundamental knowledge of endometriosis pathophysiology remains incomplete, limiting definitive conclusions about treatment. This paper is centrally about endometriosis — it specifically focuses on comparing laparoscopy with laparotomy in endometriosis treatment.

Abstract

Although the term endometriosis was first introduced in Sampson’s classic publication in 1921, much about this disease remains uncertain.1 Prevalence, etiology, and pathogenesis are still speculative and symptoms are varied. Although pelvic pain and infertility are the predominant presenting problems, endometriosis frequently produces no symptoms. It is diagnosed in more than 10% of fertile women undergoing sterilization procedures.2 Since the basic knowledge of its pathophysiology is lacking, therapy remains controversial and, in many cases, palliative. Treatment may be medical, surgical, or both. Surgery may be conservative or radical therapy, and may be performed by open laparotomy or by operative laparoscopy. Preview Unable to display preview.
