Catamenial pneumothorax caused by thoracic endometriosis
This case report describes the successful management of a recurring thoracic endometriosis case through thoracoscopic resection followed by gonadotropin-releasing hormone agonist and subsequent Dienogest treatment.

This paper reports the case of a 37-year-old woman with longstanding severe pelvic endometriosis and infertility who developed catamenial left-sided pneumothorax with symptoms recurring around menstruation. Using chest X-ray and CT/HRCT during menses, along with pelvic ultrasound and elevated CA-125, the authors identified thoracic endometriosis and performed mono-port VATS to resect pleural and lung lesions, with histology showing endometrial glands and stroma positive for estrogen receptors. The patient received chemical pleurodesis and subsequent medical therapy with GnRH agonists, and later recurrence prompted chest MRI/CT, additional GnRH agonist treatment, and prophylactic dienogest with no further thoracic or pelvic symptom recurrence during follow-up. Limitations include that this is a single case report, and it does not establish generalizable effectiveness of the diagnostic or treatment approach. This paper is centrally about endometriosis — it specifically documents catamenial pneumothorax caused by thoracic endometriosis (with concurrent severe pelvic disease).
