This article presents a unique case of synchronous extragenital endometriosis involving the rectovaginal septum, urinary bladder (dome endometrioma), and pleural/pulmonary disease in a 31-year-old woman with longstanding pelvic pain and catamenial dysuria plus recurrent, menses-associated spontaneous pneumothoraces. After prior laparoscopic endometriosis treatment, she underwent combined repeat VATS (fulguration of visible pleural endometriotic lesions followed by pleurodesis) and laparoscopic surgery with segmental bladder wall excision and repair, rectovaginal “shaving” of rectal involvement, and excision of a posterior fornix lesion. The major limitation is that the report describes a single case without histologic confirmation mentioned in the narrative and relies on operative findings and clinical correlation.
